《自然》子刊:科学家首次发现,血型会影响肠菌的组成,进而影响人体健康丨科学大发现

血型对人体健康的影响真的很谜。

去年新冠病毒肆虐期间,多个研究团队发现血型与新冠肺炎的发病或预后之间有一定的关系。甚至有好几个研究发现,O型血与感染新冠风险低[1]或者感染者呼吸衰竭风险低[2]有关,而A型血则相反

虽然观察到了这种现象,但是背后的原因究竟是什么,目前仍无人知晓。

近日,德国基尔大学Andre Franke领衔的研究团队,在著名期刊《自然·遗传学》发表了一项开创性研究成果[3],或许能让我们一窥血型的神秘。

他们通过分析9000名德国人的全基因组和肠道微生物组数据,首次确定个人基因组的遗传变异会影响肠道微生物的组成。值得注意的是,与血型相关的基因变异,竟然会影响肠道微生物的组成

尤其是一些AABB血型的人,他们的红细胞会分泌特定的糖类抗原,这些抗原进入肠道后,可能会成为特定肠道微生物的食物,促进特定肠道微生物的生长论文的第一作者Malte Rühlemann博士表示。

实际上,Franke团队一开始想要研究的并不是血型,他们真正感兴趣的还是肠道微生物。

我们都知道,进入21世纪之后,肠道微生物组被认为是多种炎症性疾病[4]、神经性疾病[5]和肿瘤性疾病[6]的重要调控因素,相关的研究那真是汗牛充栋。

但是关于人类的遗传变异对肠菌组成的影响,这些研究的结果并不统一[7-10]。好在围绕双胞胎展开的研究明确指出:个人的遗传学背景是微生物组组成的驱动因素[11]

既然如此,Franke开始提问了:人类基因组的遗传变异到底是如何影响肠菌的?这种影响与人类的健康究竟有没有因果关系呢?

带着这两个问题,Franke团队成员开始了他们的研究。

他们首先围绕微生物性状进行了一项大型的、单一国家的、全基因组关联研究(GWAS),随后进行了孟德尔随机化(MR)分析,以阐明人类与其相关微生物群之间的遗传联系。

Franke团队的研究包括5个独立的队列,它们分别是PopGen[12]n = 724),FoCusn = 957),SHIPn = 2029)和SHIP-TRENDn = 3382[1314],以及KORAn = 1864[1516]

基线数据分析显示,各队列之间的人群基本特征、基因组变异和微生物群落组成具有相似性。总的来说,各个队列人群特征差异不大,且有共同的核心微生物群落

经过进一步分析,他们在全基因组范围内鉴定出38个遗传位点与微生物特征和群落组成显著相关。

总的来看,Franke和他的同事获得的全基因组关联结果指向宿主和微生物群免疫调节的相互作用。这与我们对肠菌的认知基本相符。

举例来说,他们检测到Barnesiella spp和胆红素还原酶ABLVRArs623108)位点的变异相关

胆红素还原酶A先前已被证明能抑制TLR4基因的表达[17],而TLR-4是一种模式识别受体,可启动对革兰氏阴性细菌脂多糖的免疫反应[18]。由于革兰氏阴性菌Barnesiella sp与脂多糖诱导的干扰素的产生呈负相关,这表明Barnesiella sp通过TLR-4信号调控肠道稳态。

找到的遗传位点

Franke和他的同事感到吃惊的是,他们竟然还看到了ABO血型相关基因的身影

例如编码岩藻糖基转移酶2FUT2基因的变异与Bacteroides相关FUT2又被叫做分泌基因,与血细胞表面抗原的分泌有关,如果FUT2因突变失活,血细胞就不会分泌特定的抗原。

后续分析也发现,O型血和分泌型血与Bacteroides的流行率呈正相关。简单来说,AABB型血的人群Bacteroides丰度更高

Bacteroides之外,还有多种细菌与ABO血型或血型的分泌状态有关。总的来说,Franke和他的同事认为,ABO血型或血型的分泌状态对肠道微生物的组成有深刻的影响。

实际上,之前已经有研究发现,FUT2与炎症性肠病(IBD)相关[1920]。在本研究中,Franke团队基于孟德尔遗传分析发现,ABO血型调控的Bacteroides的存在,确实有助于限制克罗恩病的发展。另外,之前已经有研究发现,没有这种分泌途径的人感染诺如病毒的风险较低[21]

据了解,在全世界范围内,大约有20%的人属于非分泌型(包括O型血在内)[21]。如此看来,血型对肠菌的影响关系到很多人的健康。

总的来说,这项研究成果让我们对血型和肠菌以及疾病之间的关系有了新的认知,以后再研究血型与疾病之间的关系时,同时研究相关肠菌或许是个不错的研究方向。
